Select how you want to search for your formulary. If asked, type in your subscriber or group number, you can find this on your ID card.

If you don’t have your subscriber or group number, you can find your formulary by logging in to MyChart, or you can search all formularies.

The Drug Formulary Search Tool is not available for the following products:
  • Quartz Align Plan Participants
  • State and local government members

  • BadgerCare Plus members
    Please call the Department of Health and Family Services at (800) 362-3002 for information about your prescription drug benefits.
  • Medicare Advantage members who did not select a plan with Part D (prescription drug) coverage
  • Medicare Select members
  • Medicare Supplement members
