Select how you want to search for your formulary. If prompted, provide your subscriber or group number (you can find this on your ID card).

If you don't have your subscriber number or group number available to search for your exact formulary, you can:
- View all commercial formularies.
- View the Medicare Advantage formulary.
You can also view your formulary by logging in to MyChart.
